Jazmin Rivera

Chief Program, Impact & Strategy Officer

Jazmin Rivera (she/her) is the Chief Program & Strategy Officer at Partnerships for Community Health, where she leads strategy and program design to advance equity-driven outcomes across communities. She brings experience working with historically marginalized populations in both New York and New Jersey, with a focus on community voice, systems change, and population-level impact. Her leadership is guided by a deep commitment to birth equity, place-based strategies, and collective impact models that center the voices of those most affected by health inequity. As a mother of three, Jazmin approaches her work with urgency, care, and a vision for long-term change.

She holds a Doctorate of Education in Community Based Leadership from the CUNY College of Staten Island and a Master of Public Health from New York University College of Global Public Health.

Partnerships for Community Wellness is a public health nonprofit dedicated to serving Staten Island and beyond. We focus on behavioral health, maternal and child health, and emerging public health issues.
